About this course
Pain management in labor is rarely a single, planned decision. It is a series of decisions made under changing physical conditions, shifting emotional states, and evolving clinical recommendations. The parent who planned to use only non-pharmacological measures may find themselves requesting an epidural at hour sixteen. The parent who planned an epidural may arrive at the hospital in transition. Understanding how to navigate these real-time decision points — and how to process them afterward — is a distinct and important skill.
By the end of this course you will be able to apply a real-time decision framework for revising your pain management plan under labor pressure, navigate back labor with a targeted comfort measure protocol, understand the physiological and emotional dynamics that operate during augmented labor, manage unexpected epidural outcomes or placement difficulties, and process your labor pain experience as part of your birth story.
What you will learn:
- The physiological characteristics of back labor (occiput posterior position) and the specific comfort measures — counter-pressure placement, positional change, heat — most effective for it
- How the character of contractions changes with oxytocin augmentation, and how to adapt comfort measures accordingly
- Common epidural variations: partial coverage, one-sided block, window in coverage — what causes each and what can be done
- How to communicate a request to adjust epidural dosing, request a repeat dose, or report inadequate coverage to nursing staff
- The emotional dimensions of pain management decision changes: guilt about requesting pharmacological relief after a non-medicated plan, or disappointment at not experiencing labor sensation
- A framework for making real-time decisions about pain management that accounts for your current state, remaining labor duration estimates, and available options
- How fatigue and sleep deprivation interact with pain perception in prolonged labor, and how this should factor into pain management decisions
- A birth story integration exercise for processing the role of pain in your labor experience, regardless of whether the experience matched your plan
The course is built around four extended case studies: back labor managed without epidural, augmented labor with an epidural requested mid-labor, an ineffective epidural requiring troubleshooting, and a rapid labor with no time for pharmacological options. Each case is followed by guided analysis and a worksheet. The final section contains the birth story integration exercise and a guide to discussing labor pain with your postpartum care provider.
