Foundations of Labor Pain Management: Understanding Your Options

About this course
Labor pain management is one of the most consequential areas of birth preparation, yet most expecting parents encounter it as a list of options without the conceptual framework to evaluate those options against their own values, circumstances, and birth setting. Understanding how each approach works — physiologically, mechanically, or psychologically — is the foundation for making an informed, personal decision.
By the end of this course you will be able to describe the physiological mechanism and available evidence for at least six major pain management approaches, explain the trade-offs associated with each major pharmacological intervention, describe how non-pharmacological comfort measures work and in what circumstances they are most effective, and articulate your own priorities using a structured values framework.
What you will learn:
- The physiology of labor pain: why contractions hurt, how the uterus and cervix generate pain signals, and why emotional state modulates pain perception
- The epidural: mechanism, administration process, effects on labor progress, common side effects, and the evidence on outcomes
- The spinal block and combined spinal-epidural: how they differ from the standard epidural and when they are typically used
- Nitrous oxide (laughing gas): how it works, what it does and does not do, and who is most likely to find it useful
- Systemic opioids: types most commonly used in labor, their effects on pain and sedation, transfer to the newborn, and timing considerations
- Non-pharmacological comfort measures: hydrotherapy, counter-pressure, TENS units, positioning, heat and cold, movement, and breathing techniques — with an overview of the evidence for each
- Hypnobirthing and focused relaxation techniques as pain modulation strategies (see also: dedicated course on hypnobirthing)
- How to use the values-alignment worksheet to identify which approaches fit your priorities for alertness, mobility, intervention level, and partner involvement
This course is organized as five thematic readings. The first addresses the physiology of labor pain. The second covers pharmacological options in detail. The third covers non-pharmacological approaches. The fourth presents the research comparing approaches across outcome measures. The fifth guides you through the values-alignment exercise. Reflection prompts follow each reading.
