About this course
Labor does not always follow the textbook. A substantial proportion of laboring parents encounter a prolonged latent phase, a stall in active labor, a recommendation for augmentation with oxytocin, or the introduction of instrumental assistance at delivery. Understanding these variations — what drives them, what the evidence says, and what decisions they require — is a different level of preparation from understanding normal progression.
By the end of this course you will be able to apply an informed decision-making framework to augmentation recommendations, describe the evidence on common interventions for labor dystocia, understand the progression of instrumental delivery options and when they arise, and process a labor experience that included unexpected interventions or a significant divergence from your plan.

- How arrest of labor is defined and diagnosed, and the physiological causes — malposition, inadequate contraction strength, cephalopelvic disproportion — that are most frequently implicated
- The evidence base for watchful waiting versus early augmentation when labor slows, and the factors that influence this clinical decision
- How oxytocin augmentation works, what titration means in practice, and how it typically changes the character of contractions
- Positional interventions — hands-and-knees, side-lying, standing — and their role in correcting fetal malposition
- Instrumental delivery: the clinical indications for forceps and vacuum extraction, how each procedure works, and the evidence on outcomes
- How prolonged labor affects the emotional and physical state of the birthing person, and what support interventions are most relevant at that point
- How to use the BRAIN framework to evaluate intervention recommendations in real time when you are exhausted and under pressure

The course is organized around four case studies: a prolonged latent phase managed at home, an active labor stall with oxytocin augmentation, an occiput posterior malposition corrected with positional change, and a vacuum-assisted delivery at full dilation. Each case is accompanied by analysis questions, decision-point worksheets, and a resolution discussion. The final section provides a birth debrief guide and reflection prompts.
This course is suitable for expecting parents who want to prepare for labor complications as thoroughly as they prepare for uncomplicated birth, and for birth partners who need to understand these scenarios in order to support effectively.
