Understanding the Stages of Labor: Physiology, Progression, and What to Expect
Build a detailed mental map of labor from the first contractions through placental delivery — the physiological changes at each stage, what they feel like, and how labor progresses over time.
About this course
Labor is a biological process with a recognizable structure, but most expecting parents encounter it with only a vague sense of what happens in what order. Without a clear map, every unfamiliar sensation during labor raises the question: is this normal? Understanding the full physiological sequence — and the wide range of what is normal within it — reduces fear, improves decision-making, and makes it possible to communicate more clearly with care providers.
By the end of this course you will be able to describe the three stages of labor and their subdivisions, explain the physiological mechanisms that drive cervical dilation and uterine contractions, recognize the sensations and behaviors typically associated with each phase, and differentiate between normal variation in labor progress and signs that require medical attention.
What you will learn:
- The hormonal triggers that initiate labor — the roles of prostaglandins, oxytocin, and cortisol in the transition from pregnancy to active labor
- The physiological distinction between early labor (latent phase), active labor, and transition, with the typical dilation ranges and contraction patterns for each
- How effacement and dilation occur simultaneously and how care providers use these measures to assess progress
- The second stage of labor: the physiology of pushing, fetal descent through the pelvis, and delivery
- The third stage of labor: placental separation and delivery, and the management options available (active versus expectant management)
- The wide normal range for labor duration at each phase, and why comparison to average timelines is often misleading
- The emotional arc of labor — the excitement of early labor, the internal focus of active labor, the intensity of transition, and the shift at pushing — as a predictable psychological pattern
- Common variations in labor progress — prolonged latent phase, arrest of active labor — and how they are typically evaluated and managed
This course is organized as three thematic readings corresponding to the three stages of labor, preceded by a foundational reading on the hormonal initiation of labor. Each reading includes a detailed progression description, a sensory and emotional profile for the phase, and a reflection prompt. A reference summary chart compiles the key physiological markers and typical timelines across all stages for quick review.
This course is designed for expecting parents and their birth partners who want a thorough understanding of the labor process. No prior knowledge of obstetrics or biology is required. This course is informational and does not substitute for birth education with your care team or a certified childbirth educator.
