The golden hour plans made during pregnancy often meet circumstances at birth that require adaptation. A cesarean under general anesthesia, a baby who needs respiratory support, maternal hemorrhage, or a premature arrival can all alter what immediate skin-to-skin contact looks like — yet the underlying needs that the golden hour serves do not disappear in complex birth situations. Understanding how to adapt is itself a form of preparation.
By the end of this course you will be able to identify the modified forms of early skin-to-skin contact available in complex birth scenarios, apply a decision framework for prioritizing early bonding within the constraints of medical necessity, support a partner in facilitating kangaroo care when the birthing parent cannot, and develop a realistic plan for early connection in high-risk or NICU contexts.

This course is organized around four extended scenarios: a cesarean under spinal anesthesia with immediate skin-to-skin, a cesarean with unexpected general anesthesia, a premature birth with NICU admission, and a full-term birth with a postpartum maternal complication. Each scenario is followed by guided analysis questions and a planning template adapted to that context. The course closes with a framework for the postpartum debrief — reflecting on what happened, what was possible, and what was valuable.
This course is suitable for expecting parents preparing for any birth type, particularly those anticipating a cesarean, a high-risk pregnancy, or a multiple birth.
